3. What are your goals for the LSS GB course (and BB if applicable)?

Please use the Comments section to list additional goals/expectations as desired.

Complete an LSS project to become Certified (GB or BB) and lead LSS projects in the future

Be able to actively apply the LSS concepts and tools in my work role, without pursuing Certification

Understand the LSS concepts and tools in order to support others in their LSS efforts (e.g., as an LSS project team member, supervisor/manager of direct reports using LSS, etc.)

4. How would you rate your skill level for math functions that require knowledge of the order of operations (e.g., “PEMDAS,” which stands for Parentheses, Exponents, Multiplication and Division, Addition and Subtraction).

I am proficient in using the rules of PEMDAS and mathematical operations are a part of my daily work life.

I’ve applied the rules of PEMDAS in the past, (not necessarily by that term), but am rusty and may need some assistance.

I do not consider myself proficient in math (the instructor will contact you in this case, no need to panic!)

5. How would you rate your skill level in Excel for data analysis? (Check all that apply.)
